The breach affected patients from affiliates including Hamilton Emergency Medical Services, Hamilton Physician Group, Hamilton Medical Center, and Anna Shaw Children’s Institute. Exposed data may include names, Social Security numbers, addresses, dates of birth, financial account information, and medical information.
