A forensic investigation confirmed on October 21, 2024, that protected health information had been compromised, affecting 58,019 individuals nationwide. The exposed data varied but included names, medical diagnoses, prescription details, Social Security numbers, and demographic information. The breach was reported to federal and state authorities, including the U.S. Department of Health and Human Services.
