The breach, disclosed to the U.S. Department of Health and Human Services on May 29, 2025, may have exposed personally identifiable and protected health information, including names, addresses, dates of birth, Social Security numbers, driver’s license numbers, medical treatment details, and health insurance information. An investigation concluded on May 16, 2025.
